Transcript Request Information

  • Request for transcripts can be made by:

    Requests are made to Regina Richardson in the Office of Student Services.  If requesting by fax or email, be sure to include the following:

    • Your full name when you were enrolled in Surry County Schools
    • Your Date of Birth
    • Name of the Surry County School you last attended
    • Your phone number / fax number / email address
    • Name and address (or fax number) where you would like the transcript sent

    Transcripts can also be picked up at 419 N. Main Street, Dobson, NC  27017.  Proof of identity is required.
