    • Basic Procedures - $25 annual deductible (per person), paid at 70%|80%|90%|100%

    • Major Procedures - paid at 70%|80%|90%|100%

    • Maximum Yearly Out of Pocket Paid - $1,000

    The Buy-Up Option

    • Preventive Care - $0 deductible, paid at 70%|80%|90%|100%

    • Restorative Care - $25 annual deductible, paid at 70%|80%|90%|100%

    • Major Procedures - $25 annual deductible, 50% coverage

    • Maximum Yearly Out of Pocket Paid - $1,000

    Dependents are now covered up to the age of 26
